This commit adds simple implementation of guardsize for pthreads.
At this moment this option simply increases the size of allocated pthread stack.
At default pthread guard size is set to 0.

This puts the mutex support fully inside nxsem, allowing
locking the mutex and setting the holder with single atomic
operation.
This enables fast mutex locking from userspace, avoiding taking
critical_sections, which may be heavy in SMP and cleanup
of nxmutex library in the future.

There is no need for a gettid() syscall, as the thread ID is stable through
the life of the process. It is safe to put a copy of TID to TLS. This way
a user processes can access TID quickly via its own stack, instead of
having to use an expensive syscall.

After pthread mutexes changed to nxmutex, the priority inheritance
was set on by default; even if one tried to control it with
CONFIG_PTHREAD_MUTEX_DEFAULT_PRIO_INHERIT.
Also the CONFIG_PTHREAD_MUTEX_DEFAULT_PRIO_PROTECT is not effective.
Fix this by setting the default mutex priority adjustment flags according
to CONFIG_PTHREAD_MUTEX_DEFAULT_PRIO_INHERIT and CONFIG_PTHREAD_MUTEX_DEFAULT_PRIO_PROTECT.

The load/compare and RMW to wait_count need protection. Using atomic
operations should resolve both issues.
NOTE:
The assumption that the user will call pthread_cond_signal /
pthread_cond_broadcast with the mutex given to pthread_cond_wait held is
simply not true. It MAY hold it, but it is not forced. Thus, using the
user space lock for protecting the wait counter as well is not valid!
The pthread_cond_signal() or pthread_cond_broadcast() functions may be called by a thread whether or not it currently owns the mutex that threads calling pthread_cond_wait() or pthread_cond_timedwait() have associated with the condition variable during their waits; however, if predictable scheduling behaviour is required, then that mutex is locked by the thread calling pthread_cond_signal() or pthread_cond_broadcast().

In the kernel, we are planning to remove all occurrences of up_cpu_pause as one of the steps to
simplify the implementation of critical sections. The goal is to enable spin_lock_irqsave to encapsulate critical sections,
thereby facilitating the replacement of critical sections(big lock) with smaller spin_lock_irqsave(small lock)

Since pthread_mutex is implemented by sem, it is impossible to see in ps who holds the lock and causes the wait.
Replace sem with mutex implementation to solve the above problems

In order to ensure the detached thread obtain the correct return
value from pthread_join()/pthread_cancel(), the detached thread
will create joininfo to save the detached status after thread
destroyed. If there are too many of detached threads in the
process group, the joininfo will consume too much memory.
This is not friendly to embedded MCU devices.
This commit keep the semantics as #11898 was introduced,
will no longer save joininfo for detached threads to avoid wasting memory.

3. Remove the return value waiting lock logic (data_sem),
the return value will be stored in the waiting tcb.

This moves task / thread cancel point logic from the NuttX kernel into
libc, while the data needed by the cancel point logic is moved to TLS.
The change is an enabler to move user-space APIs to libc as well, for
a coherent user/kernel separation.
